Do you react when you're called that name? If so, stop reacting. =P No facial expressions of disgust or annoyance, no groans or sighs... nada. Your cousin will probably get bored of it eventually, when he concludes you don't care.

Not reacting at all is a good one, I find that I have a hard time not reacting to things. Like if my family is attempting to embarrass me and it works and I start blushing/crack a smile/twitch or something ridiculous it's game over.
However, my family is pretty frustrating with their torment. It took me a long time, but I end up learning how to deflect their comments and pretty much keep up with them. I find that with people that I've talked to, family joking around is pretty hard to stop because they think it's a) funny and b) that it isn't really hurting anyone.
But do trying ignoring him, if that doesn't work, just figure out a good enough comeback. If that doesn't work ... well ... you could attempt to throw him into a swamp like my cousin tried to do to me because of my bad attitude haha.
