School problems - September 14th 2014, 01:37 AM

I'm struggling in high school. I have anxiety, depression, and my therapist believes I might be struggling with bipolar.
I use to be on an IEP but got switched to a 504 plan last year.
I will be sitting in class and paying attention to what the teacher is talking about but then I'll totally forget what was going on. I have trouble concentrating and my mind tends to think about its own stuff when I'm paying attention in class but I sometimes wont realize it because I'm kinda use to it. I also get really stressed out when doing assignments because I get confused easily and have a hard time understanding text book talk rather then when the teacher explains something.
My psychologist said she can contact the school because she feels my issues are related to the bipolar, depression and anxiety because of how my mood fluctuates and how I get hyper then depressed.
What exactly will my teachers do if they find out about all of this? I have so much trouble with homework because after about 3 or 4 minutes I start to get overwhelmed even when I break the assignments down one by one. Will they understand more as to why I'm so stressed and give up so easily in school?
