Smoking what? Cigarettes can discolor your teeth and fingers, and even if you don't THINK so, they can make you smell. You quickly become desensitized to the odor, and might think spray and tooth paste or mints/gum will mask the smell--but they won't. If your doctor notices these signs, they might ask questions. Also, I'm fairly certain smoking causes premature aging--though I reckon you're a bit too young for that to show.

Weed can be tested for (and certainly has a distinct odor), but I reckon a doctor would first have to suspect in order to test.

Smoking either may or may not change the sound of your lungs and/or cough. I don't know how your lungs might sound to a doctor listening to a stethoscope, but I do know that a smoker's cough is quite distinct, from hearing them my entire life in family members.

You're fourteen, and while smoking is a choice, I suggest you wait until your brain development and physical growth are over with before making weed or cigarettes a habit.

Someday I will be strong enough to lift not one but both of us.
I told you to be patient
I told you to be fine
I told you to be balanced
I told you to be kind
