Best way to break up with someone? Directly. DO NOT:
- Have a friend/family member break up on your behalf.
- Use text messaging/e-mail (at LEAST do it over the phone).
- Let them know you're "done" by refusing to answer their calls/talk to them.
- Give them false hope by saying you "need a break".
- Cheat on them so you have an "excuse".

How you would say it depends on the situation. As long as you don't sugarcoat it to the point of giving the other person false hope (ex. "Maybe, someday, we can try again"), and as long as you aren't overly harsh (ex. "I'm leaving you for someone else - someone much more attractive than you!"), you're golden. =)
