Report a Staff member

Form Description

The Performance Committee monitor the activity and behaviour of members of Staff. This form can be used to submit concerns you have to them, for example if you have seen a member of Staff giving poor advice or being rude. If you have a specific complaint you would like to be addressed, for example a ban or infraction dispute, please contact the Disputes Committee instead. All concerns will be taken seriously and will be discussed carefully by members of the committee. Users will not be punished in any way for anything they say on this form.
